Know a real hero? Now it's time to let them shine.

PEOPLE and Major League Baseball are teaming up for Tribute for Heroes, a campaign honoring the nation's veterans – and is offering a once-in-a-lifetime chance to represent their favorite MLB team at the 2013 All-Star Game on July 16. Plus: One of the vets will be featured in that week's issue of PEOPLE.

Want to send the baseball-loving vet in your life to the All-Star Game? Here's how:

Nominate!
Until May 17, you can nominate an active or former member of the United States or Canadian Armed Forces at TributeforHeroes.com.

Vote!
A total of 90 finalists – that's three vets per team – will be picked by PEOPLE, MLB and a celebrity panel of judges. And then it's up to you! Vote online starting June 9 for your favorite, with the winners being announced June 10.

Tune In!
Check out the 2013 All-Star Game on FOX to see the vets honored at Citi Field during the game.

The Tribute for Heroes campaign supports Welcome Back Veterans, an organization to address the needs of vets when they return home from service.
