Hazel Fredericks, 91, a Minneapolis resident, Tuesday, in the Twin city. It was Fredericks who, in 1969, just happened to be walking down the street while TV cameras were filming Mary Tyler Moore, who at that very moment was tossing her beret into the air (for the opening title sequence of the then-new "The Mary Tyler Moore Show"). Fredericks's bemused reaction was captured forever in the credits.
